결론 및 제언
본 연구는 중환자실 입원환자중 욕창발생위험이 높은 집단에서의 욕창발생이 영양상태와
관계가 있는지를 알아보기 위하여 시도하였다. Braden scale을 이용하여 14점 이하인 경우
를 대상자로 하였으며 영양상태는 혈액화학검사와 신체계측을 이용하였다. 혈액검사내용은
혈장단백과 알부민, 혈색소이며 신체계측은 체중, 체질량, 체지방율, 무지방체질량, 체액량,
삼두박근 피하지방두께로 구성되었으며 생체전기저항을 이용하는 체지방측정기와 피부추벽
기를 이용하여 측정하였다. 자료수집기간은 97년 12월부터 98년 6월까지였으며 이기간중
Brazen score가 14점 이하이고 연구참여에 동의를 얻은 대상자는 모두 100명이었다.
연구결과는 다음과 같다.
1. 대상자의 평균나이는 55.09세이고 평균 입원기간은 15.79일이었으며 남자가 61%로 과반
수이상이었다. 신경계질환이 68%로 가장 많았고 다음이 소화기질환 10% 호흡기질환 9% 순
이었다. 욕창발생율은 17%였으며 입원후 3일이내에 가장 많이 발생하였다.
2. 욕창 발생군과 비욕창 발생군의 차이를 욕창발생 위험요인과 비교분석한 결과 욕창 발
생군에게 신체 마비와 부종 존재가 유의하게 많았고 급식하는 율이 높았으며 의식정도, 견
인 혹은 부목정도 그리고 호흡기 사용여부는 욕창발생군과 비욕창발생군사이에 유의한 차이
가 없었다.
3. 욕창발생 유무별 영양상태 차이를 분석한 결과 욕창발생군이 비욕창발생군에 비하여 체
중, 체질량지수, 무지방 체질량, 체액량, 알부민이 유의하게 낮았다.
이상의 연구결과를 바탕으로 다음의 제언을 하고자 한다.
1. 욕창발생위험집단을 대상으로 영양중재를 시행하여 그 효과를 분석하는 연구가 필요하
다.
2. 입원시부터 영양상태를 사정하여 알부민과 체중변화와 같은 선행연구에서 일관성있게
지지되고 있는 영양요소를 파악한 후 위험집단으로 분류되면 집중적인 욕창방지 간호를 시
행하는 것이 필요하다.
3. 욕창사정도구에 영양에 대한 항목을 보다 구체적으로 삽입하여 활용하는 것이 바람직하
다.

Pressure sores are a serious concerns in that respect to increasing risk of medical
complications and medical costs. Prevention and care of pressure sores is an essential
area of nursing practice. The nurse at ICU should be more careful of maintaining the
skin integrity of patients especially than at any other place. This study was conducted
to determine if the risk factors of pressure sores and nutritional status of the patients at
risk for pressure sores is related the occurance of pressure sore. The risk group refers
the patients having the below 14 scores of the braden scale. The 100 subjects were
recruited from the ICU ward at an university hospital in Choongnam. The parameters
for nutritional status are the blood chemistry including plasma protein, albumin,
hemoglobin and the anthropometric measurements consisting of weight, BMI, LBM, the
proportion of body fat, body fluid and triceps skin fold using biolmpedence analizer and
caliper.
The results are as follows :
1. The subjects were 55 years and stayed
8 days on average. Of the 100 subjects, males were 61%, neurologic/neurosurgical
diseases were 68% and the incidence of pressure sores was 17% mainly occurring
within 3days after the admission.
2. The present paralysis(or paraplegia) and edema(arm, leg, trunk) were showed more
significantly the subjects with pressure sores than those without pressure sores.
3. Regarding with the nutritional status, the subjects with Pressure sores had
significantly lower the weight, BMI, LBM, body fluid, albumin than the ones without
pressure sores.
This results were supported the reports of previous studies that the decreased weight
and albumin could be the important predictors of pressure sores. Thereafter we should
encourage these factors to be utilized in predicting pressure sores for a comprehensive
assessment. Nurse should identify patients at risk of the development of pressure sores,
assess their nutritional status and dietary intake at regular intervals.
